
Noida/UP: In Uttar Pradesh, Form-6 and Form-S are available in sufficient numbers across districts for voter list updates. These forms help add new voters, remove deceased or shifted voters, update migration details, and consolidate voters under a single booth.
SIR Update in Noida & Greater Noida
Two weeks after the launch of the Special Intensive Revision (SIR), Booth Level Officers (BLOs) have visited most households in Noida and Greater Noida. According to the District Election Officer, over 96% of the forms have been distributed. A frequently asked question remains: where to find the old voter list details and how to fill them out, especially for those without a Voter ID or EPIC number. Some citizens were advised to download the Electoral Roll PDF 2003 from the ECI website and manually select their district and assembly constituency.
Recognition for Supportive RWA and AOA
At a meeting in the Greno Authority Auditorium, the District Election Officer addressed RWAs, CREDAI, builders, BLOs, and booth-level agents regarding the SIR program. The DM emphasized zero tolerance for negligence in updating voter lists. RWAs and AOAs that provide outstanding cooperation will be officially recognized by the district administration.
Forms Available for Updates
The DM highlighted that Form-6 and Form-S are available in ample quantity, and citizens are encouraged to use them for all voter list corrections, additions, and migration updates. Suggestions or complaints related to voter lists can be sent to adeo-gbn@nic.in.

- Date of Birth (DD/MM/YYYY)
- Aadhaar Number (Optional)
- Mobile Number
- Father/Guardian’s Name
- Mother’s Name and EPIC Number
- Father/Guardian’s EPIC Number (if available)
- Husband/Wife’s Name and EPIC Number
How to Fill the Form & Required Documents
The SIR form is available at voters.eci.gov.in. Citizens must:
- Enter their EPIC number, upload a passport-size photo, and complete other personal details.
- Required documents include Aadhaar card, age and address proof, photo, and Voter ID.
- If a voter already has a Voter ID, entering the EPIC number will automatically display existing details. New voters can select the new registration option.
Steps to Verify Personal Details
- Confirm or correct existing information like name, age, gender, and address.
- Upload a recent passport-size color photograph. Face and eyes should be clearly visible.
- Apply E-signature at the designated place.
- Attach supporting documents:
- Age Proof: Birth certificate, matriculation certificate, Aadhaar, passport
- Address Proof: If changed
Once completed, submit the form online to receive an Acknowledgment Slip and Reference Number for tracking.
How to Download 2003 Voter List PDF
- Visit the ECI website via Google Search.
- Go to Voter Service Portal → Services → Search Your Name in Last SIR.
- Select State and District (e.g., Uttar Pradesh → Ballia).
- Select Assembly Constituency (AC) and Part Number, then click Show.
- Click the download link, fill in the captcha, and submit to download the PDF.
If Voters Are Not at Home
- The BLO can provide forms to an adult household member.
- If no one is present, the form will be delivered and collected again by 4 December.
- BLOs may seek information from neighbors if the voter is absent.
BLOs’ Role in Verification
- BLOs use EPIC and linked mobile numbers to contact voters.
- BLOs cannot collect documents; voters themselves provide correct information.
For Address Change
- Form 8A must be filled for shifting locations.
- Online submission: NVSP portal
- Offline submission: BLO/ER Office
- If the new address is in a different assembly constituency, use Form-6 with address proof.

BLOs are trained to assist voters in the field. If someone could not fill the form, the BLO will personally visit and help complete it.
